Alumni may order a replacement diploma by completing and mailing the following .
On the form you will provide the following:
- Your name as you attended 91³Ô¹Ï and current legal name
- Year of graduation
- Birthdate
- Provide your email address and phone number
- Address to where you wish to have the diploma sent when ready
Mail the completed form and payment to:
