Diploma Replacement Orders

Alumni may order a replacement diploma by completing and mailing the following .

On the form you will provide the following:

  • Your name as you attended 91³Ô¹Ï and current legal name
  • Year of graduation
  • Birthdate
  • Provide your email address and phone number
  • Address to where you wish to have the diploma sent when ready

Mail the completed form and payment to:

Please allow up to 6-8 weeks for processing.

Shipping:

  • USPS ($10.00)
  • FedEx Domestic ($30.00)
  • FedEx International (Please contact the Registrar’s office)
  • FedEx Overnight (Please contact the Registrar’s office)

The fee is $75.00, so make a check or money order payable in this amount to 91³Ô¹Ï and include with your completed form. Additional fees may incur for shipping international or via FedEx.

Replacement orders may take between 6-8 weeks for processing.
